Roles & Responsibilities
- Provide direct patient care with compassion — assist with activities of daily living (bathing, dressing, toileting, feeding)
- Monitor and document vital signs and patient status accurately and promptly
- Support a safe, clean environment and adhere to infection control practices
- Communicate clearly with nurses, families, and interdisciplinary team members to promote coordinated care
- Assist with mobility and safe patient transfers to preserve dignity and prevent injury
- Contribute to meaningful documentation so the care team has reliable, up-to-date information
- Respond calmly and promptly in emergencies and escalating situations
Qualifications
- Current LNA / Licensed Nursing Assistant certification (eligible to practice in NH; VT eligibility noted in posting title)
- Active, unrestricted state nursing assistant credential as required by facility
- Comfortable working night shift hours and providing weekend coverage as scheduled
- Strong interpersonal and communication skills; a team player who puts patients first
- Willingness to complete facility orientation and any required trainings
Education
- High school diploma or equivalent
- Completion of an approved nursing assistant training program and state LNA certification
Knowledge & Skills
- Hands-on patient care and assistance with ADLs
- Basic vital signs measurement and reporting
- Safe transfer and mobility assistance techniques
- Fundamental infection control and safety practices
- Clear, timely charting and communication with the care team
- Empathy, patience, and the ability to work in a fast-paced healthcare environment
