Roles & Responsibilities

• Provide compassionate hands-on care to residents, assisting with activities of daily living
• Monitor and record vital signs, reporting changes to the nursing team promptly
• Support mobility and safe transfers while using proper body mechanics
• Maintain clear, timely documentation and communicate effectively with the care team
• Follow facility policies for infection control, safety, and resident dignity at all times

Qualifications

• Active LNA (Licensed Nursing Assistant) certification — VT or NH licensure aligned with the job title
• Must be local and have reliable personal transportation (LOCAL ONLY requirement)
• Availability for per diem shifts, including evenings and potential weekends
• Comfortable working in a long-term care/rehab setting and supporting resident-centered care
• Current basic life support (BLS/CPR) certification preferred or willing to obtain

Education

• High school diploma or equivalent
• Completion of an approved LNA training program and state certification

Knowledge & Skills

• Strong basic nursing skills: ADLs, vital signs, feeding and toileting assistance
• Good documentation and communication skills with the care team
• Knowledge of infection control practices and resident safety protocols
• Ability to follow care plans and take direction from licensed nurses
• Friendly, patient-focused approach and good interpersonal skills with residents and families
