Shift Type: Monday-Friday, Day shift — one of: 07:00 AM - 03:30 PM; 07:30 AM - 04:00 PM; 08:00 AM - 04:30 PM; or 08:30 AM - 05:00 PM. Weekends required: every other weekend or 4 weekend days in a 4-week schedule. (Day shift with periodic weekend coverage)

Roles & Responsibilities

• Coordinate comprehensive discharge planning and case management for a diverse patient population
• Conduct utilization review and apply Milliman guidelines to ensure appropriate levels of care
• Document thoroughly and accurately in Epic to support continuity of care
• Collaborate with interdisciplinary teams (PT, RT, pharmacy, social services) to remove barriers to safe discharge
• Perform needs assessments and order DME as appropriate to finalize discharge plans
• Develop individualized Plans of Care and follow through on care coordination tasks
• Ensure compliance with CMS and HIPAA regulations in daily workflows
• Float within the case management service to cover gaps and ensure uninterrupted patient support

Qualifications

• Active AK Registered Nurse (RN) license in hand required for auto offer
• Minimum 1 year of nursing experience; acute care experience required
• BLS certification required
• ACM (Case Management) certification preferred
• Epic (EMR) experience required (hardstop) — strong Epic skills highly valued
• Ability to work business hours Monday–Friday with required weekend coverage
• Willingness to float within case manager services to meet facility needs
• Locals not accepted — candidate permanent address must be at least 50 miles from facility

Education

• Active Registered Nurse (RN) license required (AK RN required for auto offer)
• BLS certification required

Knowledge & Skills

• Case Management and Discharge Planning best practices
• Care Coordination across interdisciplinary teams
• Utilization Review processes and Milliman Guidelines
• Epic documentation and charting workflows
• Needs assessment and ordering DME
• Familiarity with Pediatrics, Women’s Services, ICU, SDU/PCU/IMC/Obs settings
• Understanding of CMS regulations and HIPAA compliance
• Comfort with Microsoft Office and Teams for communication and documentation
