Roles & Responsibilities

• Lead discharge planning and coordinate patient transitions to the next level of care with clarity and compassion
• Perform care coordination across units and campuses to ensure timely, safe discharges
• Conduct utilization review and needs assessments, including DME ordering and plan-of-care development
• Document thoroughly in Epic and communicate care plans to interdisciplinary team members
• Collaborate with Social Services and clinical teams to resolve barriers and improve patient flow
• Float across hospital areas (including Colby and Pacific campuses) to support unit needs and maintain continuity of care

Qualifications

• Current RN licensure (pending licenses accepted) and active BLS certification
• Minimum 1 year acute care experience with discharge planning or care coordination
• Solid case management experience — Med Surg, LLOS, and ED backgrounds are strongly preferred
• Comfortable with utilization review and CMS-related regulatory considerations
• Able to float between hospital areas and work every other weekend as scheduled
• Experience charting in Epic preferred

Knowledge & Skills

• Discharge planning and care coordination expertise
• Case management and utilization review proficiency
• Acute care clinical judgement across patient populations (neonates through geriatrics)
• Familiarity with Epic electronic health record workflows
• Strong interdisciplinary communication and patient advocacy skills
• Knowledge of CMS regulations and documentation standards
• Ability to manage higher patient ratios and complex caseloads
