Roles & Responsibilities

• Provide high-quality occupational therapy interventions to support patient independence and meaningful daily living
• Assess patient needs and develop individualized treatment plans addressing ADLs, adaptive equipment, and functional goals
• Carry out therapeutic activities and educate patients and families to promote long-term outcomes
• Collaborate closely with nurses, therapists, and other care team members to coordinate care
• Document patient progress clearly and promptly in the clinical record
• Adapt interventions based on patient response and changing clinical needs to ensure safety and effectiveness
• Support discharge planning and communicate recommendations for ongoing care

Qualifications

• Current Certified Occupational Therapy Assistant (COTA) credential — required
• Valid authorization to work in California and ability to comply with facility requirements
• Comfort traveling between assigned sites (Windsor Hampton and Stockton Nursing) as noted in the schedule
• Strong interpersonal and communication skills for team-based care and patient education
• Reliable professional presence, dependability, and commitment to patient-centered care

Education

• Completion of an accredited Occupational Therapy Assistant (OTA) program (Associate degree or equivalent preferred)

Knowledge & Skills

• Experience with ADL assessment and adaptive equipment recommendations
• Skilled in developing functional, goal-directed treatment plans
• Ability to educate patients, families and caregivers in practical strategies
• Good documentation practices and timely charting
• Strong time management and caseload prioritization
• Collaborative teamwork and effective communication across disciplines
