Roles & Responsibilities

• Provide compassionate bedside nursing care to residents, focusing on safety and dignity
• Administer medications and treatments accurately and document care in the chart
• Assess patients regularly and report changes to the interdisciplinary team
• Support ADLs and comfort measures while promoting independence
• Collaborate with RNs, CNAs and therapists to create meaningful care plans
• Educate patients and families about care, medications and follow-up needs
• Maintain clean, safe infection-control practices and accurate clinical documentation

Qualifications

• Active LPN license for practice (New Mexico) — required
• Current BLS/CPR certification
• Comfortable working night shift schedule (5:00 PM - 5:30 AM)
• Clinical experience as an LPN (preferred: prior long-term care or rehab experience)
• Strong communication skills and ability to work as part of a care team
• Ability to meet facility onboarding requirements (background check, immunizations, etc.)

Education

• Graduate of an accredited Practical Nursing program (certificate or diploma)

Knowledge & Skills

• Medication administration and safe charting practices
• Basic patient assessment and vital sign monitoring
• Familiarity with common long-term care conditions and rehab needs
• Strong documentation and electronic medical record navigation
• Infection control, wound care basics, and safe patient handling
• Patient/family education and compassionate bedside manner
