Contract Type: Travel Contract

Shift Type: Monday - Friday (5x8), Day shift. Primary: 7:30 AM - 4:00 PM. Alternative start/end options: 7:00 AM - 3:30 PM, 7:30 AM - 4:00 PM, 8:00 AM - 4:30 PM, 8:30 AM - 5:00 PM. Weekend coverage: required every other weekend (or 4 weekend days in a 4-week schedule).

Hours per Week: 40

Roles & Responsibilities

• Coordinate patient care across the continuum by leading discharge planning and care coordination efforts
• Conduct needs assessments and arrange DME and community resources to support safe transitions home
• Perform utilization review and case management activities using Milliman Guidelines when appropriate
• Document thoroughly and accurately in Epic to ensure timely, compliant patient records
• Collaborate with interdisciplinary teams (social services, PT, pharmacy, IV teams, etc.) to remove barriers to care
• Advocate for patients and families while balancing regulatory requirements (CMS, HIPAA)
• Float within the case management service as needed to cover gaps and meet facility needs

Qualifications

• AK RN license in hand required for auto-offer (screen for pending licenses)
• Minimum 1 year acute hospital RN experience (case management/utilization review experience preferred)
• Epic EMR experience required (hardstop); strong charting skills
• BLS required; ACM certification preferred
• Comfortable with intermittent weekend coverage and floating within case management scope
• Open to first-time travelers — first timers accepted
• Permanent address must be at least 50 miles from the facility (locals not accepted)

Education

• Graduate of an accredited nursing program (ADN/BSN) or equivalent preferred
• Active RN licensure (AK RN required)

Knowledge & Skills

• Strong case management and discharge planning expertise
• Utilization review knowledge and familiarity with Milliman Guidelines
• Proficient Epic charting skills and general comfort with Microsoft Office/Teams
• Ability to coordinate care across specialties including pediatrics, women's services, ICU, MS, SDU/PCU/IMC/Obs
• Excellent communication, documentation, and interdisciplinary collaboration skills
• Working knowledge of CMS and HIPAA regulatory requirements
