Contract Type: Travel Contract
Shift Type: Monday - Friday (5x8), Day shift. Primary: 7:30 AM - 4:00 PM. Alternative start/end options: 7:00 AM - 3:30 PM, 7:30 AM - 4:00 PM, 8:00 AM - 4:30 PM, 8:30 AM - 5:00 PM. Weekend coverage: required every other weekend (or 4 weekend days in a 4-week schedule).
Hours per Week: 40
Candidate Pay Rate: $74 per hour

Roles & Responsibilities
- Coordinate patient care across the continuum by leading discharge planning and care coordination efforts
- Conduct needs assessments and arrange DME and community resources to support safe transitions home
- Perform utilization review and case management activities using Milliman Guidelines when appropriate
- Document thoroughly and accurately in Epic to ensure timely, compliant patient records
- Collaborate with interdisciplinary teams (social services, PT, pharmacy, IV teams, etc.) to remove barriers to care
- Advocate for patients and families while balancing regulatory requirements (CMS, HIPAA)
- Float within the case management service as needed to cover gaps and meet facility needs
Qualifications
- AK RN license in hand required for auto-offer (screen for pending licenses)
- Minimum 1 year acute hospital RN experience (case management/utilization review experience preferred)
- Epic EMR experience required (hardstop); strong charting skills
- BLS required; ACM certification preferred
- Comfortable with intermittent weekend coverage and floating within case management scope
- Open to first-time travelers — first timers accepted
- Permanent address must be at least 50 miles from the facility (locals not accepted)
Education
- Graduate of an accredited nursing program (ADN/BSN) or equivalent preferred
- Active RN licensure (AK RN required)
Knowledge & Skills
- Strong case management and discharge planning expertise
- Utilization review knowledge and familiarity with Milliman Guidelines
- Proficient Epic charting skills and general comfort with Microsoft Office/Teams
- Ability to coordinate care across specialties including pediatrics, women's services, ICU, MS, SDU/PCU/IMC/Obs
- Excellent communication, documentation, and interdisciplinary collaboration skills
- Working knowledge of CMS and HIPAA regulatory requirements
