Roles & Responsibilities

• Deliver skilled therapeutic interventions under the direction of a physical therapist to improve patient mobility and independence
• Assist with patient assessments, implement treatment plans, and track progress toward functional goals
• Provide safe and effective patient transfers, gait training, and therapeutic exercise instruction
• Document patient care accurately and timely in the facility EMR and communicate changes to the supervising PT
• Collaborate with nurses, OT, SLP, and other team members to create cohesive, patient-centered care plans
• Educate patients and families on home exercise programs, safety, and strategies to maximize independence

Qualifications

• Current Physical Therapist Assistant license or eligibility for licensure in South Dakota
• Graduate of an accredited PTA program (required)
• Basic Life Support (BLS/CPR) certification (or willing to obtain prior to start)
• Comfortable working in a skilled nursing/long-term care setting — experience preferred
• Strong communication skills and a team-first attitude
• Ability to pass facility background checks and meet onboarding requirements

Education

• Associate degree from an accredited Physical Therapist Assistant program

Knowledge & Skills

• Knowledge of therapeutic exercise, modalities, and mobility training
• Skilled in safe patient handling and transfer techniques
• Documentation skills and familiarity with electronic medical records
• Ability to follow care plans and modify interventions under PT supervision
• Good clinical judgment, time management, and patient education skills
