Roles & Responsibilities

• Assess and evaluate residents to develop individualized treatment plans that restore function and independence
• Implement therapeutic interventions and progress care plans with clear, patient-centered goals
• Document treatment sessions thoroughly and timely to ensure continuity of care and regulatory compliance
• Collaborate with nursing, OT, speech therapy and other team members to optimize patient outcomes
• Educate residents and families on home exercise programs, safety strategies, and discharge planning
• Monitor and report changes in patient status, adjusting plans of care as needed

Qualifications

• Active Texas Physical Therapy license or eligibility to obtain licensure in Texas
• Doctor of Physical Therapy (DPT) preferred; Bachelor or Master in Physical Therapy considered
• Current CPR/BLS certification (or willing to obtain)
• Prior experience in skilled nursing, inpatient rehab, or long-term care preferred
• Strong documentation skills and ability to work collaboratively in an interdisciplinary team

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor or Master of Physical Therapy acceptable

Knowledge & Skills

• Proficient in patient assessment, therapeutic exercise, mobility and balance training
• Knowledge of rehabilitation techniques for skilled nursing and post-acute populations
• Comfortable with electronic documentation and clinical outcome measurement tools
• Excellent communication skills and patient-family education ability
• Ability to prioritize caseloads and manage time effectively in a fast-paced setting
