Roles & Responsibilities

Evaluate patients and develop individualized treatment plans that make a real difference
Provide hands-on therapy and functional mobility training to restore independence
Monitor and document patient progress with clear, timely notes
Collaborate closely with the interdisciplinary healthcare team to coordinate care
Ensure compliance with facility and regulatory standards to maintain a safe environment

Qualifications

Active California Physical Therapist (PT) license (required)
Current BLS (Basic Life Support) certification
SNF (Skilled Nursing Facility) experience preferred
Strong clinical judgment and excellent communication skills

Knowledge & Skills

Physical Therapy assessment and intervention
Patient evaluation and individualized treatment planning
Hands-on therapeutic techniques and functional mobility training
Progress monitoring and clinical documentation
Interdisciplinary collaboration and care coordination
Facility and regulatory compliance
Patient-centered care and effective communication with patients and families
