Roles & Responsibilities

• Screen, evaluate, and rehabilitate patients with communication, cognitive, or swallowing disorders using evidence-based practice
• Develop, implement, and adjust individualized plans of care in line with regulatory and clinical standards
• Use standardized assessments to measure progress and guide treatment decisions
• Document all clinical care and treatment minutes accurately in the electronic health record
• Provide consultation and counseling to patients, families, caregivers, and other providers
• Participate in patient care conferences, utilization review, and rehabilitation team meetings
• Prepare comprehensive discharge summaries and make appropriate referrals based on outcomes
• Obtain necessary DME and supplies and support smooth transitions of care

Qualifications

• Active Texas State license (required)
• ASHA Certificate of Clinical Competence (CCC-SLP) required
• Minimum 1 year of SLP experience (LTC or rehab experience required)
• New graduates accepted if they have Level II clinical experience (no Level I observation-only)
• Ability to supervise and support CFY and student SLPs
• Experience with electronic documentation / EHR and accurate clinical documentation
• CCS preferred (3 years holding active CCS is a plus)
• Strong interpersonal skills for interdisciplinary communication and patient/family counseling

Education

• Master's degree in Speech-Language Pathology or equivalent clinical training (typical pathway for ASHA CCC)

Knowledge & Skills

• Proficiency with standardized assessments and outcome evaluation
• Skilled in treatment planning, adjustment, and rehabilitation therapy approaches
• Excellent clinical documentation and electronic record-keeping
• Ability to communicate effectively with clinical teams, patients, and families
• Familiarity with regulatory and ASHA practice standards and ethical conduct
• Knowledge of DME procurement processes and discharge planning
• Experience in utilization review and participation in rehab meetings
