Roles & Responsibilities

• Lead heavy chart audits and abstractions to ensure compliance with AHA Get With The Guidelines
• Pull and verify critical time metrics from Epic to support stroke program tracking
• Ensure stroke protocol workflows are followed and coordinate process improvements
• Serve as the clinical resource for stroke patient population while not providing hands-on care
• Float between Swedish Issaquah, Redmond, and Cherry Hill (approximately 80% time at Issaquah)
• Collaborate with interdisciplinary teams to individualize stroke care plans and education
• Complete required HealthStream modules and maintain certifications to support program quality
• Screen and escalate candidate/clinical issues to manager when stroke coordinator experience is lacking

Qualifications

• Active RN license (Swedish facilities honor active compact licenses)
• Epic experience and strong chart audit/chart abstraction skills (HARD STOP)
• Experience with stroke patients and stroke program workflows
• Code response experience is required — manager will not accept first-time travelers without this
• BLS certification required
• NIHSS (NIH Stroke Scale) certification required; NIHSS can count toward stroke education hours
• 8-hour stroke education completed within the last year (department can provide modules if needed)
• Required HealthStream certifications: Code BART, Treatment Pathway, Yale Swallow Protocol, Individualizing Stroke Care Plans and Education, SNAP Stroke Neuro Assessment

Knowledge & Skills

• Proven experience with Epic and chart abstraction tools
• Familiarity with AHA Get With The Guidelines and stroke program reporting
• Strong Microsoft Office skills (Outlook, Excel, PowerPoint) for documentation and reporting
• Comfort pulling and analyzing time-based metrics from electronic records
• Ability to respond to and document code response events
• Excellent communication and interdisciplinary collaboration skills
• Experience floating across campuses and adapting to varying unit workflows
• Attention to detail and commitment to protocol-driven patient safety
