Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, hands-on patient care and daily assessments
Administer medications and treatments per care plans and policy
Monitor and report patient status changes to the care team
Document care accurately and timely in the patient record
Assist with admissions, discharges, and safe patient transfers
Collaborate with RNs, CNAs, and interdisciplinary team members to optimize outcomes

Qualifications

Active LPN license in Pennsylvania (required)
Current CPR/BLS certification (or willingness to obtain)
LPN diploma or certificate from an accredited program
Experience in direct patient care; experience in long-term care or similar settings preferred
Strong communication skills and the ability to work well in a team

Education

LPN diploma or certificate
High school diploma or GED

Knowledge & Skills

Medication administration and documentation
Basic wound care and dressing changes
Patient assessment, vital signs, and reporting
Familiarity with Electronic Medical Records (EMR) documentation
Infection control and patient safety practices
Effective communication and time management skills
