Roles & Responsibilities

• Perform comprehensive outpatient physical therapy evaluations and develop individualized treatment plans
• Deliver hands-on therapy, therapeutic exercises, and functional training to help patients regain mobility and independence
• Document patient progress accurately and timely in the electronic medical record to ensure continuity of care
• Collaborate with interdisciplinary team members to coordinate patient-centered care
• Educate patients and caregivers on home exercise programs, safety, and prevention strategies
• Adapt treatment approaches for patients across outpatient, acute, and skilled nursing settings based on clinical presentation

Qualifications

• Current California Physical Therapy License
• Passed the National Physical Therapist Examination (NPTE)
• Current Healthcare Provider BLS Certification (AHA)
• Minimum of 1 year of experience in outpatient, acute, swingbed/skilled nursing, or related settings
• Proficient in patient documentation and comfortable using clinical computer systems
• Strong interpersonal skills, punctuality, and commitment to compassionate care

Education

• Bachelor of Science in Physical Therapy

Knowledge & Skills

• Up-to-date knowledge of physical therapy treatment practices and modalities
• Excellent interpersonal communication and compassionate patient care
• Analytical thinking and problem-solving to tailor effective interventions
• Ability to manage varied diagnoses and multi-task clinical responsibilities
• Timely and professional written communication and accurate documentation
• Comfortable using computers and clinical software
