Candidate Pay Rate: $85 - $59.5 per hour

Roles & Responsibilities

Evaluate residents and create individualized physical therapy treatment plans that promote mobility and independence
Deliver hands-on therapy focused on strength, balance, gait, and functional recovery
Monitor patient progress, update care plans, and adjust treatments to achieve measurable outcomes
Collaborate with physicians, occupational therapists, nursing staff, and interdisciplinary teams to ensure coordinated care
Maintain accurate, timely clinical documentation and rehab records
Educate residents and caregivers on exercises, mobility techniques, and fall prevention strategies to support long-term success

Qualifications

Active California Physical Therapist license required
Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
Previous experience in SNF, rehabilitation, or geriatric therapy preferred
Strong clinical assessment and patient care skills with an emphasis on functional outcomes
Excellent communication skills and the ability to work effectively within a multidisciplinary team

Education

Doctor of Physical Therapy (DPT) or equivalent from an accredited program

Knowledge & Skills

Expertise in mobility training and rehabilitation techniques
Strong patient evaluation and treatment planning abilities
Proficient clinical documentation and electronic record keeping
Effective communication with patients, families, and care teams
Skilled in fall prevention strategies and caregiver education
