Roles & Responsibilities

• Assess, plan, and implement nursing care for residents under RN supervision, ensuring continuity and quality
• Administer medications and pass meds accurately after completing the required med competency test
• Supervise and mentor CNAs and support staff to maintain safe, resident-centered care
• Document resident care clearly and timely in the EMR to keep the care team informed
• Perform resident assessments and contribute to individualized care planning
• Cross-train in other nursing departments as needed to support unit needs
• Communicate professionally and empathetically with residents, families, and agency representatives

Qualifications

• Current California LVN license required
• Minimum Skilled Nursing Facility (SNF) experience — sub-acute does not count
• Current BLS certification required
• Successful completion of medication competency test (med pass) required
• Comfortable supervising CNAs and coordinating with interdisciplinary teams
• Basic computer skills and ability to use Electronic Medical Records preferred
• Strong English verbal and written communication skills

Education

• Vocational Nursing diploma or equivalent and active California LVN license

Knowledge & Skills

• Medication administration and medication safety practices
• Resident assessment and care planning for long-term care populations
• Skilled Nursing (SNF) clinical workflows and documentation
• Electronic Medical Records navigation and documentation best practices
• BLS protocols and emergency response readiness
• Leadership and supervision of CNAs and support staff
• Professional interpersonal skills for residents, families, and staff interactions
