Roles & Responsibilities

• Perform thorough evaluations to assess residents' physical abilities and needs
• Design and implement individualized treatment plans that promote independence
• Deliver evidence-based physical therapy interventions to improve strength, mobility and function
• Educate residents and families on therapeutic exercises, safe movement techniques and home modifications
• Collaborate with interdisciplinary team members—OTs, nurses and physicians—to coordinate comprehensive care
• Monitor patient progress and adjust treatment plans to optimize outcomes
• Maintain accurate, timely documentation of assessments, progress notes and treatment plans in accordance with facility policy
• Ensure care plans align with regulatory requirements and best-practice standards

Qualifications

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program
• Valid California Physical Therapy license (required)
• Experience in a skilled nursing facility or similar healthcare setting preferred
• Strong communication and interpersonal skills with a compassionate, patient-focused approach
• Ability to work independently and as part of a collaborative team
• Familiarity with therapeutic modalities, adaptive equipment and mobility improvement strategies

Education

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy (accredited program)

Knowledge & Skills

• Skilled in patient evaluation, assessment and individualized treatment planning
• Experience with therapeutic modalities and therapeutic exercises
• Knowledge of adaptive equipment and home modification strategies
• Proven ability to document care clearly and accurately
• Strong interdisciplinary collaboration and care coordination skills
• Excellent problem-solving and communication abilities
