Roles & Responsibilities
- Provide compassionate direct nursing care to residents, promoting dignity and comfort
- Administer medications and treatments accurately and document per facility policy
- Assess residents, monitor changes in condition, and communicate findings to the team
- Collaborate with nurses, therapists, and support staff to deliver coordinated care plans
- Assist with admissions, transfers, and discharges while maintaining clear documentation
- Support rehabilitation goals by encouraging participation in therapy and daily activities
- Follow infection control and safety protocols to protect residents and staff
Qualifications
- Active Massachusetts LPN license required; active MA RN license accepted if applying as RN
- Current BLS/CPR certification required
- Completion of an accredited LPN program (or RN program if applicable)
- Previous experience in long-term care or rehab settings preferred but not required
- Strong communication skills and ability to work within an interdisciplinary team
- Reliable, patient-centered, and comfortable with electronic charting systems
Education
- Graduate of an accredited LPN program
- High school diploma or equivalent
- BLS/CPR certification
Knowledge & Skills
- Solid medication administration and documentation practices
- Basic assessment skills and ability to recognize changes in condition
- Familiarity with long-term care and rehabilitation nursing principles
- Good infection control practices and safety awareness
- Comfortable using electronic health records and basic clinical tech
- Strong interpersonal skills and compassionate bedside manner
