Roles & Responsibilities

• Provide compassionate direct nursing care to residents, promoting dignity and comfort
• Administer medications and treatments accurately and document per facility policy
• Assess residents, monitor changes in condition, and communicate findings to the team
• Collaborate with nurses, therapists, and support staff to deliver coordinated care plans
• Assist with admissions, transfers, and discharges while maintaining clear documentation
• Support rehabilitation goals by encouraging participation in therapy and daily activities
• Follow infection control and safety protocols to protect residents and staff

Qualifications

• Active Massachusetts LPN license required; active MA RN license accepted if applying as RN
• Current BLS/CPR certification required
• Completion of an accredited LPN program (or RN program if applicable)
• Previous experience in long-term care or rehab settings preferred but not required
• Strong communication skills and ability to work within an interdisciplinary team
• Reliable, patient-centered, and comfortable with electronic charting systems

Education

• Graduate of an accredited LPN program
• High school diploma or equivalent
• BLS/CPR certification

Knowledge & Skills

• Solid medication administration and documentation practices
• Basic assessment skills and ability to recognize changes in condition
• Familiarity with long-term care and rehabilitation nursing principles
• Good infection control practices and safety awareness
• Comfortable using electronic health records and basic clinical tech
• Strong interpersonal skills and compassionate bedside manner
