Roles & Responsibilities

Provide compassionate, high-quality direct patient care as an LPN on the unit
Administer medications and treatments accurately and document thoroughly
Monitor patient status and communicate changes promptly to the care team
Assist with admissions, discharges, and transfers to ensure smooth transitions
Support RNs and interdisciplinary teams with clinical tasks and patient education
Maintain a safe, clean, and respectful environment for patients and colleagues

Qualifications

Current and active LPN license in Pennsylvania (required)
Basic Life Support (BLS) or CPR certification (required or willing to obtain)
Comfortable with medication administration, vitals, and basic clinical documentation
Strong communication skills and a team-oriented mindset
Previous experience in long-term care, skilled nursing, or similar settings a plus

Education

Practical Nursing diploma or equivalent from an accredited program

Knowledge & Skills

Patient assessment and basic clinical troubleshooting
Medication administration and IV/SC basics where applicable
Charting and electronic health record familiarity
Infection control, safety protocols, and patient hygiene care
Clear verbal and written communication with patients, families, and staff
