Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents consistent with care plans
• Administer medications and treatments safely and accurately
• Monitor resident status and promptly report changes to the care team
• Maintain clear, timely documentation in the medical record
• Assist with ADLs and support resident independence and dignity
• Collaborate with RNs, therapists, and support staff to deliver coordinated care
• Participate in unit-level quality initiatives and safety practices

Qualifications

• Current Licensed Practical Nurse (LPN) credential in good standing
• Basic Life Support (BLS/CPR) certification (or willingness to obtain)
• Comfortable with medication administration, wound care, and routine assessments
• Strong communication skills and a team-first attitude
• Previous experience in long-term care, skilled nursing, or related setting preferred

Education

• Graduate of an accredited practical nursing program
• High school diploma or GED required

Knowledge & Skills

• Solid knowledge of clinical nursing fundamentals and resident care practices
• Medication administration and documentation best practices
• Infection control and safety procedures
• Empathetic patient/resident interaction and family communication
• Basic familiarity with electronic medical records and charting
• Ability to prioritize tasks and manage time during an 8-hour shift
