Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and treatments safely and accurately per facility protocols
• Monitor and document patient vital signs, changes, and care progress
• Collaborate with RNs, CNAs, and interdisciplinary team members to coordinate care
• Assist with ADLs (activities of daily living) and promote patient comfort and dignity
• Communicate clearly and kindly with patients, families, and care team members

Qualifications

• Current Licensed Practical Nurse (LPN) license — Pennsylvania licensure preferred/required
• Recent experience working as an LPN in long-term care, skilled nursing, or similar setting is a plus
• Current CPR/BLS certification (or willingness to obtain upon hire)
• Clear background check and ability to meet facility onboarding requirements
• Strong communication skills and a patient-centered attitude

Education

• Graduate of an accredited Practical Nursing (LPN/LVN) program
• High school diploma or GED

Knowledge & Skills

• Solid knowledge of basic nursing principles and medication administration
• Comfortable with charting and accurate documentation
• Ability to assess patient needs and escalate concerns appropriately
• Team-oriented approach and good time management in a fast-paced care setting
• Compassionate bedside manner and respect for resident dignity
