Roles & Responsibilities

Provide compassionate, patient-centered care following facility policies
Administer medications and treatments accurately and document thoroughly
Perform routine assessments and report changes in patient condition promptly
Collaborate with nurses, therapists, and providers to support care plans
Maintain accurate and timely documentation in the medical record
Assist with activities of daily living to promote patient comfort and dignity

Qualifications

Current Licensed Practical Nurse (LPN) license (Pennsylvania license or eligibility preferred)
Completion of an accredited LPN/LVN program
Basic Life Support (BLS/CPR) certification (or ability to obtain)
Comfortable with medication administration and basic clinical procedures
Strong communication skills and ability to work in a team
Ability to pass required background checks and health screenings

Education

Graduate of an accredited LPN/LVN program; high school diploma or equivalent

Knowledge & Skills

Patient assessment and vital signs monitoring
Medication administration and documentation
Wound care and basic clinical procedures
Familiarity with electronic health records and charting
Infection control practices and patient safety principles
Compassionate bedside manner and time management
