Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM (Day) with every-other-weekend rotation (Weekend rotation required)
Hours per Week: 40
Candidate Pay Rate: $75 per hour

Roles & Responsibilities
- Coordinate patient care across the continuum — inpatient care coordination from admission to safe discharge
- Lead discharge planning and arrange DME, SNF placements, and ambulance transport when needed
- Assess patients and organize/participate in family meetings to align goals and plans
- Develop and document individualized treatment plans in Epic with attention to quality and regulatory standards
- Collaborate with interdisciplinary teams, HMOs/IPAs, and external partners to support transitions of care
- Navigate appeals, denials, and workers' compensation processes as appropriate
- Ensure compliance with CMS, DRG, HIPAA, OSHA and The Joint Commission standards
- Maintain clear, timely charting/documentation and support continuity of care for adults and geriatric patients
Qualifications
- Valid California state license required (pending license accepted only if fully licensed by assignment start)
- Minimum 2 years acute medical Care Manager experience performing care coordination (HARDSTOP on travel experience — travel experience required)
- Proven experience using Epic for documentation and care coordination
- Comfortable working with a 1:25 patient ratio and supporting adult/geriatric populations
- Weekend rotation availability required (every-other-weekend coverage)
- Experience with discharge planning, DME/SNF/ambulance arrangements, and family meetings
- Familiarity with appeals/denials, disability case management, and disease management preferred
- Strong communication skills and ability to work within interdisciplinary teams
Knowledge & Skills
- Epic electronic health record expertise and computerized documentation
- Care coordination and discharge planning best practices
- Charting/documentation accuracy and regulatory documentation
- Assessment of adult and geriatric patients and treatment plan development
- Arranging DME, SNF placement, and ambulance transport independently
- Knowledge of CMS, DRG, HIPAA, OSHA, and The Joint Commission requirements
- Experience with appeals/denials, workers' compensation case management
- Familiarity with Emergency Department, ICU, Medical-Surgical and Surgical Services settings (preferred)
