Roles & Responsibilities

• Perform comprehensive speech-language evaluations to determine patient needs
• Provide daily treatments and individualized therapy interventions
• Plan and execute safe, timely discharges in coordination with the care team
• Document progress accurately and maintain productivity expectations
• Collaborate with nursing and interdisciplinary team members to optimize care
• Follow CRG policies & procedures to ensure compliance and quality of care

Qualifications

• Valid SLP credentials/license in the state of New Mexico or eligibility to obtain one
• 1-3 years of clinical experience (SNF experience strongly preferred)
• Comfort being onsite to receive guaranteed hours and meet productivity targets
• Ability to follow facility policies (CRG P&P) and meet a 90% productivity requirement
• Professional appearance: black scrubs per facility dress code

Knowledge & Skills

• Strong skills in SLP evaluations, treatment planning, and discharge planning
• Experience providing therapy in a skilled nursing facility preferred
• Solid documentation skills and timely charting practices
• Excellent communication and teamwork to coordinate with interdisciplinary staff
• Ability to manage caseload and meet productivity expectations
