Roles & Responsibilities

• Evaluate patients' functional abilities and design individualized OT treatment plans
• Provide skilled therapeutic interventions to improve independence with ADLs
• Progressively document patient status, outcomes and discharge planning
• Collaborate with interdisciplinary team members to coordinate care and goals
• Train and educate patients and caregivers on adaptive techniques and equipment
• Monitor and modify treatment plans based on patient response and team input
• Advocate for patient needs and support transitions to the next level of care

Qualifications

• Current, active Occupational Therapy license in Michigan or eligibility to obtain one
• Degree in Occupational Therapy from an accredited program
• Experience working in a rehabilitation or skilled nursing facility is highly desirable
• Strong clinical assessment and documentation skills
• CPR certification or willingness to obtain upon hire
• Excellent communication and teamwork abilities

Education

• Degree in Occupational Therapy (Bachelor's, Master's, or entry-level OTD depending on state requirements)

Knowledge & Skills

• Skilled in ADL assessment, adaptive equipment recommendation and therapeutic activities
• Knowledge of SNF documentation standards and rehab workflows
• Proficient in clinical reasoning, goal setting and measurable outcome tracking
• Comfortable collaborating with nursing, therapy aides and case management
• Ability to prioritize caseloads and manage time effectively
