Roles & Responsibilities

• Ensure safe and effective medication therapy by reviewing orders and monitoring patient regimens
• Prepare and verify unit dose and IV admixtures with precision and adherence to protocols
• Manage drug distribution operations to maintain accurate, timely delivery to clinical areas
• Collaborate closely with care teams to optimize pharmacotherapy and patient outcomes
• Deliver teaching and precepting for staff and learners to elevate practice across the department
• Conduct and contribute to research and quality improvement projects that improve care
• Maintain compliance with State and Federal laws, regulations, and hospital policies
• Communicate clearly and professionally with patients, families, and interdisciplinary teams

Qualifications

• Active pharmacist license in Vermont or eligibility to obtain licensing prior to start
• Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy preferred
• Hospital pharmacy experience required; retail experience alone will not meet requirements
• Hands-on experience with unit dose systems and IV admixture preparation
• Strong computer skills and familiarity with pharmacy information systems
• Excellent communication skills and the ability to be self-directed in clinical practice

Education

• Doctor of Pharmacy (PharmD) preferred; Bachelor of Pharmacy accepted

• Clinical pharmacy practice and therapeutic decision making

• Unit dose systems and IV admixture compounding
• Hospital pharmacy operations and regulatory compliance
