Roles & Responsibilities

• Provide compassionate direct patient care and assist with activities of daily living (ADLs) such as bathing, dressing, and toileting
• Assist with safe transfers and mobility, using proper body mechanics and equipment
• Take and record vital signs and report changes in patient condition to the nursing team
• Document care accurately and timely in the patient record to support continuity of care
• Maintain a safe, clean, and therapeutic environment while following infection control protocols
• Collaborate with nurses, therapists, and support staff to help implement individualized care plans
• Support residents and families with dignity and respect, offering emotional support when needed
• Participate in unit huddles and follow assigned assignments at the Wayne center

Qualifications

• Current Certified Nursing Assistant (CNA) certification (state-issued) required
• Ability to obtain or maintain any facility-required vaccinations and background checks
• Comfortable working a rotating schedule including every other weekend and holiday rotations
• Strong communication skills and ability to work within an interdisciplinary care team
• Reliable, compassionate, and detail-oriented with a commitment to resident safety

Education

• Completion of an approved CNA training program
• High school diploma or GED preferred

Knowledge & Skills

• Knowledge of ADLs, basic patient care, and proper transfer techniques
• Basic vital sign measurement and documentation skills
• Familiarity with infection control practices and PPE usage
• Proficient in clear, professional communication with residents and care team members
• Time management, prioritization, and the ability to follow care plans
• Respect for resident privacy, dignity, and HIPAA standards
