Roles & Responsibilities

• Provide compassionate direct patient care and assist with activities of daily living
• Monitor and report changes in patient condition to the nursing team promptly
• Perform personal care tasks such as bathing, dressing, and mobility assistance
• Document care accurately and timely to support continuity and quality
• Support a safe, clean environment by following infection control and safety protocols
• Collaborate with nurses and interdisciplinary team members to deliver excellent resident-centered care

Qualifications

• Current LNA license (NH or VT) — required
• CPR/BLS certification (current) preferred or obtain upon hire
• Comfort working in long-term care or skilled nursing environments
• Ability to pass background checks and required health screenings/immunizations
• Reliable, patient-centered attitude with strong communication skills

Education

• Completion of a state-approved LNA training program or equivalent
• High school diploma or GED preferred

Knowledge & Skills

• Knowledge of basic patient care techniques and ADL assistance
• Skill in taking and recording vital signs accurately
• Good documentation and time-management skills
• Ability to work well within a care team and follow care plans
• Understanding of infection prevention and safety practices
