Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in the rehab/skilled nursing setting
• Administer medications and treatments accurately and safely
• Monitor patient status and communicate changes to the care team promptly
• Document assessments, interventions, and care outcomes clearly in the medical record
• Collaborate with nurses, therapists, and providers to deliver person-centered care plans
• Assist with wound care, ADLs, and restorative nursing tasks to promote resident independence
• Support a safe environment by following infection control and safety protocols

Qualifications

• Active Licensed Practical Nurse (LPN) license — New Mexico licensure preferred or eligibility to obtain
• Current BLS (Basic Life Support) certification required
• Experience in long-term care, skilled nursing, or rehab settings preferred
• Strong clinical assessment, medication administration, and documentation skills
• Ability to work the assigned night shift schedule (Thu-Fri-Sat) and collaborate with a team

Education

• Practical Nursing diploma or equivalent completion of an accredited LPN program

Knowledge & Skills

• Knowledge of medication administration and side-effect monitoring
• Familiarity with wound care and restorative nursing techniques
• Proficient charting and clear clinical documentation
• Strong communication and teamwork skills with residents and interdisciplinary staff
• Ability to prioritize tasks during busy overnight shifts and respond to changing needs
• Basic proficiency with electronic health record systems
