Roles & Responsibilities

• Provide high-quality direct patient care with compassion and professionalism
• Administer medications and treatments according to facility protocols
• Monitor and document vital signs and patient status accurately in the chart
• Collaborate with RNs and interdisciplinary team members to follow care plans
• Perform wound care and basic clinical procedures within scope of practice
• Communicate clearly with patients and families, educating them on care and treatment
• Assist with admissions, discharges, and transfers to ensure smooth transitions

Qualifications

• Current LPN license (eligible to practice in West Virginia; state requirements apply)
• Completion of an accredited LPN/Vocational Nursing program
• Basic Life Support (BLS) / CPR certification (preferred or required per facility)
• Previous LPN clinical experience in a long-term care or skilled nursing setting preferred
• Able to pass facility background checks and required health screenings

Education

• Graduate of an accredited LPN/Vocational Nursing program; state licensure required

Knowledge & Skills

• Strong clinical assessment skills and attention to detail
• Medication administration and documentation proficiency
• Comfortable with electronic health record charting
• Good communication and teamwork abilities
• Time management and prioritization during busy night shifts
• Basic wound care and restorative nursing skills
