Roles & Responsibilities

• Conduct comprehensive clinical screening, assessment, diagnosis, and treatment planning for patients across the lifespan.
• Deliver brief therapy and continuing care interventions to support patient recovery and resilience.
• Lead and co-facilitate patient care conferences and coordinate discharge planning for acute, ALF, or transitional placements.
• Provide care coordination and client advocacy to ensure smooth transitions across the continuum of care.
• Identify and facilitate referrals for chemical dependency and mental health services as needed.
• Maintain accurate, timely, and compliant documentation to meet state, federal, accreditation, and billing standards.
• Collaborate with hospital teams and community partners, including tribal health entities, to optimize patient outcomes.
• Participate in trainings, quality management activities, and help integrate best practices into service delivery.

Qualifications

• Masters Degree in Social Work, Psychology, Counseling, or related discipline from an accredited institution
• Licensed or in the process of becoming licensed as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage & Family Therapist (LMFT)
• Valid State of Alaska driver’s license or ability to obtain within 6 months of hire
• Must be 21 years of age
• Clinical competency to be achieved within 3 months of hire and revalidated every three years
• Completion of SEARHC Clinical Competency Assessment (required)

Education

• Master's degree in Social Work, Psychology, Counseling, or related field

Knowledge & Skills

• Discharge planning and care coordination best practices
• Clinical assessment, treatment planning, and brief therapy techniques
• Crisis intervention and interviewing techniques
• Documentation standards for medical charting and billing compliance
• Case management and interdisciplinary team collaboration
• Cultural competency in tribal health and working with diverse populations
• Understanding of ethical, legal, and accreditation requirements
