Roles & Responsibilities
- Provide high-quality, compassionate nursing care to residents at Applewood Rehab Ctr
- Administer medications and treatments accurately and document per policy
- Monitor resident status and communicate changes promptly to the care team
- Assist with care planning and participate in multidisciplinary team rounds
- Support residents with activities of daily living while promoting independence
- Maintain accurate, timely clinical documentation and ensure regulatory compliance
- Mentor and support CNA staff when appropriate to elevate overall care quality
Qualifications
- Active Licensed Practical Nurse (LPN) license (able to practice in New Hampshire preferred)
- Current BLS/CPR certification or willingness to obtain prior to start
- Valid, up-to-date immunizations and mandatory facility credentials
- Ability to work the scheduled 7:00 AM - 7:30 PM day shift and reliably commute to Winchester
- Strong communication skills and a collaborative, patient-centered mindset
Education
- LPN diploma or certificate from an accredited nursing program
Knowledge & Skills
- Medication administration and accurate charting
- Fundamental nursing assessments and resident monitoring
- Infection control practices and safe patient handling
- Time management, prioritization, and delegation
- Clear communication with residents, families, and interdisciplinary teams
- Basic rehabilitation and long-term care familiarity
