Roles & Responsibilities

Provide high-quality, compassionate nursing care to residents at Applewood Rehab Ctr
Administer medications and treatments accurately and document per policy
Monitor resident status and communicate changes promptly to the care team
Assist with care planning and participate in multidisciplinary team rounds
Support residents with activities of daily living while promoting independence
Maintain accurate, timely clinical documentation and ensure regulatory compliance
Mentor and support CNA staff when appropriate to elevate overall care quality

Qualifications

Active Licensed Practical Nurse (LPN) license (able to practice in New Hampshire preferred)
Current BLS/CPR certification or willingness to obtain prior to start
Valid, up-to-date immunizations and mandatory facility credentials
Ability to work the scheduled 7:00 AM - 7:30 PM day shift and reliably commute to Winchester
Strong communication skills and a collaborative, patient-centered mindset

Education

LPN diploma or certificate from an accredited nursing program

Knowledge & Skills

Medication administration and accurate charting
Fundamental nursing assessments and resident monitoring
Infection control practices and safe patient handling
Time management, prioritization, and delegation
Clear communication with residents, families, and interdisciplinary teams
Basic rehabilitation and long-term care familiarity
