Roles & Responsibilities

• Provide safe, patient-centered medication services through order entry and verification
• Check and dispense medications accurately to support inpatient care
• Rotate into ambulatory roles such as anticoagulation clinic or oncology when scheduled
• Collaborate with nurses, providers, technicians, and other care team members to optimize therapy
• Ensure compliance with state and federal pharmacy laws in all dispensing and documentation
• Support care across populations from pediatrics to geriatrics with clinical judgment
• Communicate medication-related recommendations clearly to improve patient outcomes

Qualifications

• Current Pennsylvania pharmacy license OR eligible for reciprocation in Pennsylvania within 90 days
• Bachelor of Science in Pharmacy required; Doctor of Pharmacy (PharmD) preferred
• Recent hospital pharmacy experience preferred
• Pharmacy residency and advanced practice certifications are a plus for ambulatory rotations
• BLS (Healthcare Provider) required for anticoagulation clinic rotations, renewed every two years
• Able to work evenings and weekends, collaborate with multidisciplinary teams, and work with minimal supervision

Education

• Bachelor of Science in Pharmacy (required)
• Doctor of Pharmacy (PharmD) (preferred)

Knowledge & Skills

• Strong knowledge of state and federal pharmacy laws and regulations
• Proficient in order entry, medication verification, and dispensing workflows
• Excellent attention to detail and medication safety practices
• Effective interprofessional communication and teamwork skills
• Experience with inpatient pharmacy operations and ambulatory clinic practices
• Ability to manage multiple patient populations and prioritize care tasks
