Hours per Week: 36

Candidate Pay Rate: $28 per hour

Roles & Responsibilities

Provide high-quality bedside nursing care to patients in the rehab and long-term care setting
Administer medications and treatments safely and accurately per physician orders
Monitor patient status and communicate changes promptly to the care team
Document assessments, interventions and patient responses clearly and thoroughly
Support patient mobility, ADLs and participation in therapeutic activities
Collaborate with nurses, therapists and support staff to deliver coordinated care

Qualifications

Current Licensed Practical Nurse (LPN) credential (required)
Active state nursing license in good standing or eligible to obtain one
Basic Life Support (BLS) certification preferred or willing to obtain
Experience in rehab, long-term care or similar patient populations is a plus
Strong communication and teamwork skills with a patient-first attitude
Ability to work the posted schedule (Mon–Wed, 6:00 AM–6:30 PM)

Education

Completion of an accredited Practical Nursing (LPN) program

Knowledge & Skills

Strong skills in medication administration and wound care basics
Comfortable with documentation in electronic health records
Ability to perform patient assessments and recognize changes in condition
Good time management during extended shift hours
Compassionate patient-centered communication and family engagement
Team-oriented approach to interdisciplinary care
