Roles & Responsibilities

• Provide skilled physical therapy evaluations and develop individualized treatment plans
• Deliver hands-on therapy, therapeutic exercise, and functional training to support patient recovery
• Collaborate with multidisciplinary teams to coordinate patient-centered care
• Document patient progress clearly and timely to ensure continuity and quality of care
• Educate patients and families on home programs, safety, and discharge planning
• Monitor outcomes and adjust interventions to maximize patient function and independence

Qualifications

• Valid Massachusetts Physical Therapy license (or compact license where applicable)
• Current Basic Life Support (BLS) certification
• Experience in skilled nursing, rehab, or post-acute care settings preferred
• Strong clinical reasoning, documentation skills, and teamwork mindset
• Excellent communication skills with patients, families, and interdisciplinary staff

Education

• Doctor of Physical Therapy (DPT) preferred; Bachelor’s or Master’s in a related field considered

Knowledge & Skills

• Skilled in physical therapy assessment, therapeutic exercise, and mobility training
• Knowledge of post-acute care protocols, assistive devices, and fall prevention
• Ability to create progressive, measurable treatment plans
• Comfortable with electronic medical records and timely documentation
• Strong interpersonal skills and the ability to work well in a team-based environment

This is a travel contract in the beautiful Berkshires.
