Roles & Responsibilities

• Evaluate patients and develop individualized treatment plans to restore function and independence
• Provide targeted therapy focused on activities of daily living (ADLs) and functional goals
• Treat patients recovering from illness, injury, or surgery with evidence-based techniques
• Monitor patient progress and adjust care plans to maximize outcomes
• Educate patients and caregivers on home exercise programs and safe routines
• Collaborate closely with nursing and rehab team members to coordinate care
• Maintain accurate, timely documentation that supports continuity of care

Qualifications

• Active California Occupational Therapist (OT) license required
• SNF/LTC experience preferred and a plus for hit-the-ground-running care
• Strong communication and teamwork skills to collaborate across disciplines
• Comfortable with documentation and electronic health record workflows
• Available for an immediate start or short notice to meet patient needs

Knowledge & Skills

• Skilled in ADL-focused therapy and functional independence strategies
• Ability to assess, plan, implement, and re-evaluate individualized care plans
• Proficient in patient/caregiver education and home exercise program development
• Strong interdisciplinary collaboration and clinical documentation skills
• Compassionate, patient-centered approach with good clinical judgment
