Roles & Responsibilities

Provide compassionate direct patient care that promotes dignity, comfort, and safety
Administer medications and treatments accurately and document thoroughly
Perform routine assessments and communicate changes in patient condition to the care team
Assist with activities of daily living while encouraging patient independence
Maintain clear, timely charting in the electronic medical record
Collaborate with nurses, therapists, and physicians to deliver coordinated care

Qualifications

Active Licensed Practical Nurse (LPN) license in Pennsylvania required
Basic Life Support (BLS) certification required or willing to obtain
Current immunizations and ability to pass background and drug screening
Comfortable working with diverse patient populations and communicating with families
Dependable, punctual, and committed to high standards of patient care

Education

Completion of an accredited practical nursing program
High school diploma or equivalent required

Knowledge & Skills

Strong skills in medication administration and documentation
Knowledge of vital signs monitoring and basic clinical assessments
Familiarity with infection control practices and patient safety standards
Ability to use electronic medical records and complete accurate charting
Excellent interpersonal and communication skills
Team-oriented approach with good time management and organization
