Roles & Responsibilities

Provide direct patient care and monitor patient status with compassion and attention
Administer medications and treatments per physician orders and facility protocols
Document patient care activities accurately and timely in the medical record
Collaborate with the interdisciplinary team to support patient care plans
Assist with patient admissions, discharges, and transfers to ensure smooth transitions
Educate patients and families on care plans, medications, and discharge instructions
Respond promptly to patient needs and escalate changes in condition to nursing leadership

Qualifications

Current and active LPN license in Pennsylvania
BLS/CPR certification required; other certifications a plus
Reliable professional with good clinical judgment and strong communication skills
Previous direct patient-care experience preferred but new grads with strong skills are welcome
Ability to pass facility-required background checks and health screenings

Education

Diploma or certificate in Practical Nursing (LPN) from an accredited program

Knowledge & Skills

Fundamental nursing assessment and observation skills
Medication administration and documentation best practices
Familiarity with electronic health records and accurate charting
Basic wound care and vital signs monitoring
Strong interpersonal skills and teamwork
Time management and ability to prioritize patient needs
