Roles & Responsibilities

• Provide compassionate, hands-on personal care including assistance with activities of daily living
• Monitor and document patient vital signs and changes in condition promptly and accurately
• Assist nursing staff with safe patient transfers, positioning, and mobility support
• Maintain a clean, safe environment and follow infection control protocols
• Communicate clearly with patients, families, and the interdisciplinary care team
• Support resident dignity and comfort while promoting independence whenever possible

Qualifications

• Current LNA (Licensed Nursing Assistant) certification in VT or NH as required
• Valid state-required certifications and ability to work in the assigned state(s)
• Comfortable working 12+ hour day shifts and on a rotating weekend schedule
• Strong interpersonal skills and a patient-centered approach
• CPR/BLS certification preferred or willingness to obtain if required by facility
• Prior experience in long-term care or acute care settings is a plus

Education

• High school diploma or GED preferred
• Current LNA certification (required)

Knowledge & Skills

• Proficient in assisting with ADLs (bathing, dressing, toileting, feeding)
• Knowledge of basic infection control and safety procedures
• Accurate documentation skills and basic charting familiarity
• Ability to measure and record vital signs and report changes
• Good communication, teamwork, and time-management skills
