Roles & Responsibilities

Provide compassionate direct patient care, following care plans and physician orders
Monitor resident conditions and report changes to the care team promptly
Administer medications and treatments safely and accurately
Document observations, interventions, and patient responses thoroughly
Collaborate with RNs, CNAs, therapists, and leadership to deliver coordinated care
Assist with admission, discharge, and transfer processes to ensure smooth transitions
Educate residents and families about care plans, medications, and self-care as appropriate

Qualifications

Current LPN license (eligible to work in Pennsylvania preferred)
Current Basic Life Support (BLS) certification or willingness to obtain
Prior experience in long-term care, skilled nursing, or similar setting preferred
Strong communication and teamwork skills with a resident-centered approach
Ability to perform clinical tasks typical to LPN scope of practice

Education

Completed accredited Practical Nursing / LPN program (diploma or certificate)

Knowledge & Skills

Clinical assessment and vital sign monitoring
Medication administration and documentation
Basic wound care and infection control practices
Charting in electronic or paper medical records
Time management and prioritization in a fast-paced care setting
Ability to work collaboratively within an interdisciplinary team
