Roles & Responsibilities

• Provide direct physical therapy services to residents under the direction of a licensed Physical Therapist
• Implement and follow the physician's plan of care, documenting patient progress clearly and professionally
• Operate within the scope of the state Physical Therapy Practice Act to ensure safe, compliant care
• Collaborate with nursing and interdisciplinary teams to optimize patient outcomes and discharge planning
• Educate patients and families on safe mobility, therapeutic exercises, and techniques to support recovery

Qualifications

• PTA credential or PTA graduate eligible for state licensure; able to work under Physical Therapist supervision
• Comfortable providing direct patient care in a Skilled Nursing Facility (SNF) setting
• Open to all experience levels — any experience accepted, including new graduates

Knowledge & Skills

• Hands-on physical therapy techniques and rehabilitation practices used in SNF care
• Direct patient care skills with strong documentation habits
• Familiarity with the Physical Therapy Practice Act and working within PT supervision
• Ability to follow physician orders and contribute as part of an interdisciplinary care team
