Roles & Responsibilities

• Treat patients under the direction of the Physical Therapist, delivering compassionate, evidence-based care
• Record daily treatment notes and weekly progress summaries in accordance with PT Board requirements
• Follow physician treatment plans and adjust care delivery as directed by the therapist
• Assist with maintaining an organized, safe and well-equipped treatment area
• Participate in patient care and rehabilitation conferences to advocate for patient progress
• Support nursing staff by training Restorative Aides and caregivers on maintenance programs
• Lead in-service training sessions to share knowledge and improve team skills
• Report equipment issues promptly to ensure patient safety and uninterrupted care
• Communicate clearly with supervisors and interdisciplinary team members about patient progress and care plans
• Instruct patients, families, and nursing staff in home programs and discharge preparations

Qualifications

• Licensed Physical Therapist Assistant in good standing with the state board
• Ability to meet continuing education requirements per state practice
• Proficient computer skills and experience inputting data into Casamba and PointClickCare
• Strong communication skills and ability to collaborate with interdisciplinary teams
• Experience delivering patient-centered care in a rehabilitation or long-term care setting
• Comfort training and mentoring Restorative Aides and nursing staff

Education

• Licensed Physical Therapist Assistant
• Meets state continuing education requirements and maintains license in good standing

Knowledge & Skills

• Patient care and rehabilitation techniques
• Treatment delivery per physician and therapist plans
• Accurate documentation and progress note writing per PT Board standards
• Caregiver training and patient/family education
• Restorative Aide training and in-service instruction
• Equipment maintenance awareness and reporting
• Electronic medical record systems: Casamba and PointClickCare
• Strong interpersonal skills and professional communication
