Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents and patients
- Take and record vital signs, monitor condition changes, and report findings
- Administer medications and treatments accurately and safely per policy
- Maintain clear, timely documentation in patient charts and care plans
- Collaborate with RNs, interdisciplinary team members, and leadership to support patient outcomes
- Assist with admissions, discharges, and smooth transitions of care
- Promote infection prevention best practices and a safe care environment
- Educate patients and families with empathy and clarity about care plans
Qualifications
- Active LPN license in Pennsylvania or eligible for licensure
- Current BLS (Basic Life Support) certification or ability to obtain
- Comfortable with medication administration, charting, and basic nursing procedures
- Strong communication and teamwork skills, with a patient-focused attitude
- Previous long-term care, skilled nursing, or acute care experience preferred
Education
- Graduate of an accredited Practical/Vocational Nursing program (LPN)
Knowledge & Skills
- Clinical assessment and basic nursing interventions
- Medication administration and documentation best practices
- Familiarity with patient charting and electronic health records
- Infection control and safety procedures
- Time management and prioritization in a fast-paced care setting
- Effective communication with patients, families, and care teams
