Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents/patients in accordance with facility policies
Administer medications and treatments accurately and document in the chart
Perform routine patient assessments and promptly report changes in condition
Assist with activities of daily living and mobility as needed
Collaborate with RNs, CNAs, and interdisciplinary team members to coordinate patient care
Maintain accurate, timely documentation and follow infection-control protocols
Support quality improvement initiatives and contribute to a positive care environment

Qualifications

Current, active LPN (Licensed Practical Nurse) license in Pennsylvania or eligible to obtain
BLS/CPR certification (or ability to obtain prior to start)
Strong communication and interpersonal skills; patient-focused mindset
Ability to pass background check, drug screen, and required facility onboarding
Previous experience in long-term care, skilled nursing, or similar setting is a plus

Education

Graduation from an accredited Practical Nursing (LPN/LVN) program; high school diploma or equivalent

Knowledge & Skills

Medication administration and accurate charting
Basic wound care and infection control practices
Vital sign monitoring and patient assessment
Electronic medical record (EMR) documentation familiarity
Time management, prioritization, and teamwork in a fast-paced environment
