Candidate Pay Rate: $56 per hour

Roles & Responsibilities

• Provide skilled physical therapy evaluation and treatment to home health patients
• Assess functional status, mobility, balance and safety in the home environment
• Develop individualized care plans focused on restoring independence and function
• Document interventions and progress clearly in the patient record (Epic preferred)
• Collaborate with nurses, case managers and caregivers to coordinate care
• Participate in required competency testing and maintain compliance with immunizations and screenings

Qualifications

• Current, unrestricted Physical Therapy licensure (verification required)
• Basic Life Support (BLS) certification
• Home health experience or strong interest in community-based care
• Epic experience preferred but not required
• Successful completion of Trinity specialty competency testing and skills checklist
• Up-to-date immunizations and required health screenings (see checklist)

Knowledge & Skills

• Strong assessment and clinical reasoning skills for outpatient/home health populations
• Skilled in therapeutic interventions for mobility, balance and functional independence
• Proficient documentation skills and familiarity with EHR systems (Epic preferred)
• Excellent communication and patient/caregiver education abilities
• Time management and caseload prioritization in a community setting
• Ability to work independently and collaborate with interdisciplinary teams
