Contract Type: Per-Diem

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments accurately and on schedule
Monitor and document vital signs, reporting changes to the RN or provider
Collaborate with the interdisciplinary team to support care plans
Maintain accurate, timely charting in the patient record
Educate patients and families about care plans and daily routines
Assist with admissions, discharges and transfers to ensure smooth transitions

Qualifications

Active LPN (Licensed Practical Nurse) license in Pennsylvania
Completion of an accredited practical nursing program
Current BLS/CPR certification or willingness to obtain upon hire
Ability to pass background check and drug screening per facility policy
Strong communication and teamwork skills; patient-centered mindset
Previous long-term care or skilled nursing experience preferred but not required

Education

High school diploma or GED
Completion of an accredited practical nursing program (LPN certificate/diploma)

Knowledge & Skills

Medication administration and documentation best practices
Basic patient assessment and vital sign monitoring
Familiarity with electronic medical records and charting
Infection control and standard precautions
Time management and prioritization in fast-paced clinical settings
Effective communication with patients, families and clinical teams
