Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate bedside care to residents with dignity and respect
Administer medications and treatments accurately and on schedule
Monitor and record vital signs and report any changes to the nurse in charge
Assist with activities of daily living including hygiene, mobility and feeding
Collaborate with the care team to develop and follow individualized care plans
Document patient care thoroughly and maintain accurate medical records
Support admissions, discharges and safe patient transfers
Participate in shift handoffs to ensure continuity of care

Qualifications

Valid LPN license in Pennsylvania required
Current BLS/CPR certification required or must obtain upon hire
Up to date immunizations and ability to pass background checks
Comfortable working in long term care or skilled nursing settings
Strong communication skills and a team-first attitude
Previous LPN experience preferred but new graduates with clinical experience are welcome

Education

LPN diploma or equivalent from an accredited program

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and infection control principles
Patient assessment and vital sign interpretation
Electronic charting or paper charting experience
Time management and the ability to prioritize tasks
Collaborative teamwork with nurses, CNAs and therapy staff
