Roles & Responsibilities

• Perform comprehensive initial and ongoing nursing assessments across physical, psychosocial, environmental, and community domains
• Develop, initiate, and modify individualized Plans of Care; coordinate with physicians and the care team
• Complete OASIS data and documentation according to federal, state, and agency regulations
• Provide skilled nursing care including wound/ostomy care, infusion management, catheter and tube feeding support
• Deliver patient and family education to promote safe recovery and chronic disease self-management
• Establish and communicate discharge plans with patients, physicians, therapists, and caregivers
• Supervise and coordinate CNA activities and collaborate with PT/OT/SLP for therapy needs
• Perform telehealth/virtual visits in addition to in-home care to enhance access and continuity

Qualifications

• Active Illinois RN license (copy required with submission)
• Current BLS certification (must be active at time of submission)
• Valid driver’s license and active car insurance (required due to travel between homes)
• Home Health experience required (Med-Surg experienced candidates with some Home Health will be considered)
• OASIS and Start of Care experience strongly preferred
• COVID and influenza vaccination required — please include vaccination status with your profile
• Two supervisory references and current resume required for submission
• Active certifications and license documentation must be available at time of submission

Knowledge & Skills

• Thorough knowledge of home health nursing processes, OASIS completion, and start-of-care workflows
• Strong documentation skills and adherence to regulatory guidelines
• Care coordination and discharge planning across disciplines
• Experience with telehealth and virtual visit delivery
• Skill in wound/ostomy care, infusion management, diabetes care, catheter/tube feeding
• Patient/family education techniques and chronic disease management
• Use of EMR systems (Matrix provided) and mobile documentation tools
• Patient safety practices including SAFE error prevention habits
