Roles & Responsibilities

• Provide compassionate, culturally sensitive diabetes education to patients and families, translating complex care into doable steps
• Utilize the nursing process to assess, plan, implement and evaluate individualized patient care plans
• Administer prescribed medications and treatments safely and in line with state law and facility policies
• Document care accurately and according to standards to ensure continuity and quality of care
• Collaborate with an interdisciplinary diabetes management team to support longitudinal patient outcomes
• Act as a resource for staff on patient education materials and program development
• Participate in unit performance improvement and shared governance activities to drive better care
• Advocate for patients addressing clinical, psychosocial and care-coordination needs

Qualifications

• Licensed Registered Nurse (RN) in Illinois (IL) and Wisconsin (WI) — both required
• CDCES (Certified Diabetes Care and Education Specialist) — required
• Approved CPR certification (obtainable within 90 days of hire)
• Graduate of an accredited nursing program (BSN preferred)
• Comfortable administering medications, developing care plans, and documenting per standards
• Willingness to float to other units and collaborate within multidisciplinary teams
• Strong critical thinking and patient advocacy skills

Education

• Graduate of an accredited nursing program (BSN preferred)

• Diabetes education techniques and patient/family teaching strategies
• Clinical assessment and the nursing process for individualized care planning
• Medication administration and safe practice standards
• Documentation best practices and clinical record-keeping
• Interdisciplinary collaboration and communication
