Roles & Responsibilities
- Provide compassionate, culturally sensitive diabetes education to patients and families, translating complex care into doable steps
- Utilize the nursing process to assess, plan, implement and evaluate individualized patient care plans
- Administer prescribed medications and treatments safely and in line with state law and facility policies
- Document care accurately and according to standards to ensure continuity and quality of care
- Collaborate with an interdisciplinary diabetes management team to support longitudinal patient outcomes
- Act as a resource for staff on patient education materials and program development
- Participate in unit performance improvement and shared governance activities to drive better care
- Advocate for patients addressing clinical, psychosocial and care-coordination needs
Qualifications
- Licensed Registered Nurse (RN) in Illinois (IL) and Wisconsin (WI) — both required
- CDCES (Certified Diabetes Care and Education Specialist) — required
- Approved CPR certification (obtainable within 90 days of hire)
- Graduate of an accredited nursing program (BSN preferred)
- Comfortable administering medications, developing care plans, and documenting per standards
- Willingness to float to other units and collaborate within multidisciplinary teams
- Strong critical thinking and patient advocacy skills
Education
- Graduate of an accredited nursing program (BSN preferred)

- Diabetes education techniques and patient/family teaching strategies
- Clinical assessment and the nursing process for individualized care planning
- Medication administration and safe practice standards
- Documentation best practices and clinical record-keeping
- Interdisciplinary collaboration and communication
- Performance improvement and shared governance principles
