Roles & Responsibilities

• Perform thorough patient evaluations to form effective, individualized treatment plans
• Deliver daily treatments focused on safe, efficient rehabilitation and discharge to home
• Manage and document patient progress, treatment notes, and discharge plans accurately and timely
• Collaborate with the interdisciplinary rehab team to optimize patient outcomes
• Prioritize rehab-to-home pathways for appropriate patients and support transitions of care
• Meet facility productivity expectations and follow CRG policies and procedures

Qualifications

• Licensed Physical Therapist (PT) — New Mexico license or ability to obtain prior to start
• Doctor of Physical Therapy (DPT) credential preferred (PT credentials accepted)
• 1-3 years of clinical experience (as listed in the posting)
• Prior skilled nursing facility (SNF) experience strongly preferred
• Ability to work on-site (therapist must be present in the building to receive guaranteed hours) and follow facility dress code (black scrubs)
• Willingness to comply with CRG policies and meet productivity requirements (90%)

Education

• Doctor of Physical Therapy (DPT) preferred
• Physical Therapy degree / qualifying PT credential required for licensure

Knowledge & Skills

• Skilled in patient evaluations, daily treatments, and discharge planning
• Experience working in SNF/skilled nursing environments and rehab-focused caseloads
• Strong documentation and time-management skills to meet productivity targets
• Collaborative communication with nursing, social work, and therapy colleagues
• Ability to prioritize rehab-to-home strategies and support patient independence
